Mobile Care Practice

Through multiple sites of service we provide weekly physician wound management to achieve successful healing outcomes.


  • Supplement Home Health wound patients with our care services to improve healing times with more frequent visits and dressing changes without sacrificing patients' other care needs.
  • We provide onsite consultation and treatment for Home Health, Skilled Nursing Facilities, Long-term Care, Long-term Assisted Centers, Hospice and Assisted Living.

Care Coordinating

  • AWC coordinates schedules and treatment plans to best support fiscal viability while treating patients' wounds effectively.
  • Our providers communicate with healthcare partners to ensure quality of care between visits.
  • All physician notes and diagnoses are ready immediately following weekly rounds for PDPM documentation integration with your facility's EHR system.

  • Assist in navigating State Inspection requirements
